VOLUNTARY HEALTH INSURANCE in the National Health Fund (NFZ) – Wadowicka 8W street<\/strong><\/p>\n\n\n\n

  1. The application (received at the MCK PK) with attachments should be submitted to the Journal of the National Health Fund (Wadowicka 8W street<\/strong>).<\/li>\n\n\n\n
  2. After submitting the application, you should wait for information about the date of signing and collecting the contract. The identification number of the application will be given in the e-mail or by phone – it should be provided when picking up the contract.<\/li>\n\n\n\n
  3. To pick up the contract at the entrance, please take a ticket from the machine (\u201cumowy dobrowolne\u201d) and go to the Info. Please bring your passport and the Pole’s card (if you have one).<\/li>\n\n\n\n
  4. After receiving the Contract, it should be presented at the MCK PK secretariat.<\/strong><\/li>\n<\/ol>\n\n\n\n

    During holidays and non-working days, medical assistance is provided by night and holiday health care points.<\/p>\n\n\n\n

    Night and holiday health care are basic health care benefits provided from Monday to Friday from 6.00 p.m. to 8.00 a.m. the next day and on non-working days around the clock. Below is a list of facilities on duty in Cracow :<\/p>\n\n\n\n
