Voluntary health insurance
VOLUNTARY HEALTH INSURANCE in the National Health Fund (NFZ) – Wadowicka 8W street
- The application (received at the MCK PK) with attachments should be submitted to the Journal of the National Health Fund (Wadowicka 8W street).
- After submitting the application, you should wait for information about the date of signing and collecting the contract. The identification number of the application will be given in the e-mail or by phone – it should be provided when picking up the contract.
- To pick up the contract at the entrance, please take a ticket from the machine (“umowy dobrowolne”) and go to the Info. Please bring your passport and the Pole’s card (if you have one).
- After receiving the Contract, it should be presented at the MCK PK secretariat.

During holidays and non-working days, medical assistance is provided by night and holiday health care points.
Night and holiday health care are basic health care benefits provided from Monday to Friday from 6.00 p.m. to 8.00 a.m. the next day and on non-working days around the clock. Below is a list of facilities on duty in Cracow :
